I am wondering what word you would use to tell the patient you need to chart?
For example "don't flush the toilet, we need to chart the urine amount."

I can ditto this. I'm a teacher in a bilingual city (El Paso, Texas) and I have heard and seen notar or tomar nota de... used in this manner, for a variety of situations where information or data needs to be recorded. Registrar could probably be used, but that seems like a very formal way of speaking to me, like what you'd see in an instruction manual.

Instead of "tomar nota", I would say "no tire de la cadena, tenemos que controlar la cantidad de orina"
I don't know about mexicans, but here in Spain we use "tirar de la cadena" (pull the chain) to talk about flushing the toilet, because back in the day most of the WCs had a top deposit (usually over the head), and there was a chain you should yank to open the conduct, and flush the toilet. You can also use "no use la cisterna" instead.
I think "controlar la cantidad de orina" is better, in this case, than "tenemos que anotar la cantidad de orina", which is other possibility.
