This. I remember when I started to read extensively in English (and then later in other languages) that after a certain point I stopped reaping the benefits form that in terms of vocabulary acquisition. I started to incorporate vocabulary review time and then and after a while I noticed a drastic difference. And then later, I familiarized with the statistical analysis of texts and discovered the statistical distribution in a typical book. Now I think that you need to have some form of vocabulary review, if you want your vocabulary to grow at the advanced stages at a steady rate. (Doesn't mean that I myself very systematic with that though.)
